Title: Need help finding this bait. Bought 15years ago off tv. Having a mold made.

This is my best top water bait, but I am now out of them.  This is the only bait I have ever used that can be used waitless and woke as a plug.  Problem is I can not find them anywhere.  I am having it drawn in cad and then a mold made to make my own.  If you know what they are please let me know. Thanks
